Oteracil
Oteracil ist ein Wirkstoff, der die Aktivität der Dihydropyrimidin-Dehydrogenase im Gastrointestinaltrakt hemmt, um die systemische Toxizität von fluoropyrimidinbasierten Chemotherapeutika wie Tegafur zu reduzieren. Es wird hauptsächlich in Kombinationspräparaten zur Behandlung von Magenkrebs eingesetzt, indem es die Nebenwirkungen der Chemotherapie im Verdauungstrakt verringert.

Anwendung
Oteracil ist Teil des Kombinationspräparats Teysuno, das zusammen mit Tegafur (einem Prodrug von 5-Fluorouracil) und Gimeracil für die Behandlung von fortgeschrittenem Magenkrebs bei Erwachsenen in Kombination mit Cisplatin verwendet wird.
Wirkmechanismus
Oteracil wirkt durch die Hemmung des Enzyms Dihydropyrimidinase, das im Gastrointestinaltrakt lokalisiert ist. Dieses Enzym ist für den Abbau von 5-Fluorouracil (5-FU) verantwortlich. Indem Oteracil die Aktivität dieses Enzyms hemmt, reduziert es die Umwandlung von 5-FU in seine inaktiven Metaboliten im Darm, was zu einer verringerten gastrointestinalen Toxizität führt.
Gleichzeitig beeinflusst Oteracil nicht die Umwandlung von 5-FU in aktive Metaboliten in Tumorzellen, wodurch die antitumorale Wirksamkeit von 5-FU erhalten bleibt.
